Case outline

The doctors’ team, which during the 90s was engaged in the Gender Conforming Process. That modest expression is usually, mentally, a familiar guide to a minimum “harassment and disturbance “of immediate trans and homo-phobic surrounding communities where MTF girls were born and grew up, within which they are happy just to be able to peacefully live. There are many reasons why a case presented after twenty-seven (whole life) years is still an old case report. As a specific patient, first with the fully opened high school transition and almost fairy-tale history of treatment, there is a significant deviation from the normal form pursued.

Case report with presentation of important details

This Case Report aimed to indicate how, as a transgender patient, one should not need to try at all to change sexual drives, as much as it is, was, or exists at all. This was recklessly impudent and a bit rude. But those are just a few of the defending mechanisms. More and more complicated, it was more than it is (here it is unnecessary), but equipped with a snake almost nasty, rude MO of addressing the main number of people, for example, spiking with – becoming a live social network was a handy trick to get it in the sleeve. Just tried to defend myself with that poisoned sentence and word combination.

Results with today’s existing consequences: Today myself without only my sister seeing her and her children, do not speak with my mother, and for now I have no father’s trust at all. Friends are something that is for now, good, it is not a topic. Pretending made me and I stayed dangerous, nobody loves my company, and I really have trouble loving and trusting. As said, no one tried any emotion on me for more than a decade.

It is unfortunately not the case in all cases, even though they passed almost all the same stages, and compared to my patients, they were even more conscious patients, simply within a young enough age, where it is more suitable to be called simply a child, and genetically, still, some of us could remember such events, emotionally, especially delicate. Case report

Life here is experienced and in the form of the Case report of the Belgrade Team for Gender Identity, with reference to the diagnostic schedule recommended by the Standards of Care for Dysphoric Persons of the Harry Benjamin International Gender Dysphoria Association [9]. From elementary school in a small town near the capital as a straight A student, old (7 years old) as a little pioneer (here last generation took the sharps’ and be prepared to swear in ceremonial custom, with the most solemn reception and school performance, with one’s yearly ceremony. It is almost impossible, and just need to picture it, when such a small and fragile, sexless child, victimized already, OMG – squeaky like a pet’s rubber toy, yeah, little boy, has such a brave performance constantly. Forced to play some “they think supposed to be a boy’s games”, was something that some amount had always been subject to, and also took an effort. That kind of dedication and commitment started to be an inspiration for others. The teacher usually used examples of miracles she was astonished to help the others, and it was often subject to respect, no minoring other pupils, and with no evil will, there always were and wished to tutor them, which really was an inspiration, an option of freedom to for some role choosing a role for some girls. It was clear for the urgency of what should be done. As soon as possible. The reaction of parents at first was turbulent but not threatening, just a strange mixture of emotions predominantly. One day, two psychiatrists came to lunch. Father was yelling, and mother cried for a long time. Traumas happened, if someone wants but cannot help, and just insults their parents. After that clumsy experiment, occasional sadness started to feel like something normal. Special children early figured out their destiny. The importance was because of not showing any signs of hesitation or willpower changing. When anyone normal would be forced to realize how this kind of plan is socially obscene, neither parent was warned by anyone. Everything, without signs of fear of any kind, was just a bravery show off, never tortured infant, even from within itself demanded respect. Prudent and confident, it stayed away from anybody who tried to influence its decision, even with the memory of the previous year or awareness, for sure made that some kind of this kind of survival strategy one of the primary occupations for further. During the ages of 14 and 15, stopped growing taller, and at 173 cm “brakes” in sky rocket, moving direction, also were welcomed. Naturally gifted with “rarely, one in a million on a biological male’s pretty face”, also never in a masculine way, like something which surprises me even, and all body and face begin to form a super-puzzle. It was enough to put my hair and nails to grow, use almost invisible makeup, and just being from a small city society already stop to looked strange creature – all started to fit. Those years were like the time needed to wait for some exotic flower to open. Like characters from many stories, starting going through a process of changing, and then being brutally ridiculed, even my body tried to delay it, but puberty nevertheless started. Then came the realization that everything is so terribly wrong. Everything was wrong. Such body fat starts melting, so-called breast contours vanish. Ugly facial hair is starting to appear, and something else is more and more night mare. My parents always knew that something was wrong, but now, they were considered.

Survey – one on one

At first, thought that must which could happen is that all is some kind of hermaphroditism, because it cannot be so strong and so true until a year or two in an empty head, everything starts to be remembered as more and more bad thoughts. Pictures with feminine body parts are, even it is there or not, constantly present in such imaginations. Only children suffering now from something similar, that offal symptoms of transgender disorder self-recognition could know that bitter sense, the creepy nature of that strange pain. Precisely, the pain of not being in a proper “package” really sounds even funny now. No, it was all but for smiling then. Gender is a subjective feeling of one’s sex, and therefore, transsexuality is an extreme form of gender dysphoria [10]. In clinical practice, the concept of gender identity and gender identity disorders (GID) has three main components: gender identity, gender role, and sexual orientation [11,12].

The relationship between gender identity and sexual orientation is important for clinicians dealing with GID, as well as for scientists exploring general human behaviours. It is important to note that the sexual orientation of persons with GID could be homosexual, heterosexual, bisexual, or asexual. Clinical experience has shown that, in comparison to non-erotic manifestations, eroticism per se is far less manifested in persons with GID [13]. But I was not suffering alone and desperate for a long time. In that time, it is usual to practice wearing first very precise make up lines, getting almost un visible face new contour look, mixture of for that fashion intersex outfit combinations, diving hair, doing nails…when all supposed are ready to celebrate sweet sixteen, fake but very realistic it is possible to be readier for such a birthday than in American teen movie. Very early many friends, neighbours and relatives taking such serious girl, more, seriously more and more. In a second and a half year of hormone therapy, the prettiest girl in school, like from a joke – it was not at all hard to become. How? That was the nineties in something that was country made by leftovers of the former Yugoslavia. The complexity of transgender phenomena is just verified by this story with special and strongconfirmation from getting the idea that „whole” town had some opinion of me, and as sexual attractive appearance, many groups formed at the end literally revered me as a real celebrity, openly discussed and shared it (there was no Facebook in 1996). Naturally, there were several smaller groups of young people, “hanging” on playgrounds, or in parks and their favourite blocks in the evening, (high school boys in general, but there were a few exceptions), and for sure it was scary and made me literally petrified by passing through, without my “bodyguard’s friends. No matter that I was some kind of teenage star in 30 000 populated small town near the capital, the neo-Nazi movement was starting to strengthen, and the popular called “Skinheads” at that time would always shout derogatory sending me massage of threatening. Walking till then, brave, with a high nose point to the sky, walking single was a habit need to be changed. They were the socio-cultural phenomena multiplying, and even Serbia in that time was torn apart by civil war, international isolation, hyperinflation, and living there was by the same rules as in some western movies. During that last decade of the previous century, Serbia was subject to serious social changes. Those changes were similar to those that happened in other Eastern European countries, but bear some unique features. In Serbia, the processes of transition were slowed down and misled by the aggressive politics of Slobodan Milosevic. Nationalistic and militant moods dominated Serbia for the entire decade, but the consequences were not only what the regime expected – so looking whole discourse, but also a small number. Things in which really someone like me is being forced to even listen, no God to listen to real authorities. While society and its norms were falling apart, the spirit of anarchy and disobedience was getting stronger, eventually leading to the political changes. Since this was a very diverse process, this article is only on certain elements. Already described what some parties were turning for, but the attitudes recognized as “pro – western”, of younger generations were evolving, independent, in a positive way. My generation will never forget that personal perspective as an active one, unless touched by the active role in the process of political and cultural changes. Shortly, but enough and important for this article, it is important to mention that together with many of my friends, the most important thing was listening and making good music, basic for the new Serbian culture, which was very global in its nature. Also, managing with time without knowing what else is going to exist, by foot in small time invented “live social network”. With the same rules, with those placed stories which was arrange to be in just favour in places where they needed to be. It was important to be just quick, rude, and brave, “freely said” anything about anybody, and bossing around the whole city, more and more people begged for my friendship, anything was possible by just being close to friends, who are also close, or directly close to places where it was planned to be. Remembering those periods, when life is not your own at all, but all should or must be like that. When the transition was over, I would know how to tell if I was wasting energy, such as in small hydroelectric power plants. Still, very tired, people, but more stubborn people found me, also from the margins of society. Mere sex for money, for some reason, back then didn’t mean much; there was a lot of it, so parties were organized all over the district where people could freely be a couple. And suddenly it started, the guys realized about 30 min. Beautiful girls around me every day on different occasions. They had their parties (those also were little younger groups of boys, football players, parents with money, spoiled boys). They desperately needed something possible to be organized in a few minutes, imagine something to stop me in. A good period of time started. In the third year of high school, in a way be just a student, and already leading a half-criminal organization, it is not, and it is far from whatever will be regular, will be positive. Desperate times, same like that measures, that was a handy catch phrase…Cute boys hired me to approve the hottest girls in the city, and they found the places. Every Saturday, with a rich clientele on the edge of the law, but with a job done, became a little queen B of tables in a great discotheque. The girls were there, never forced into anything; people from the criminal milieu were satisfied because they found love. Organization of everything, in the evening, at 18, normally, my normal look is chosen to run a semi-illegal organization. Early tomorrow, going to school. On Mondays finally started to play music on the popular radio show Rock Workshop and improve my pop culture. A syndrome of burning out was threatening to me -it is funny, but it is serious now, but no one saw neither one ray of it. At first sight, he happened, boyfriend right in front of the SRS was throwing glass on it, and I saw him. He didn’t touch me until the end, and we are still married. My life started. Was, and it is now the life of someone who is one in a million.

Discussion

In Serbia, the whole TG situation still follows the “no freak police” procedure. “No or later hard pass and usual and soon seek from depression Women” are not allowed to even start the transition. Hurt women, at every step, will become depressed and sick, which is just an assumption. No freak police is something we called it then, and it still is. Blackmailed into finding and enrolling in a faculty - i.e., starting higher education and only then being released “for surgery”. Same today, and honestly, don’t know if it’s ethical, but we’re all mostly were so, so happy, and there are no failures or those who are shouted at on the street. Serbia won’t do that to people. Although it’s cruel, we all agreed with it. Through some of the most beautiful photos, and you’ll think that wasn’t fair to the rest. Seriously, our people deserved it all.

Generation my friends from class were within, could be called the “anti” generation. Around me, and during transition as a circus for old, old people, and also fun, fun for little ones - they knew to through chest nuts, so being famous, on both sides, vertically, or horizontally be famous as anti – moralistic generation allowed to be and do so many new one west seeing new no important news – tattoos and piercing. Many of us were used to practical ways of survival, and didn’t pay attention to ethical values anymore. Didn’t care about other people as long as they were not threatened. This was the perfect secure conditions in a country such as Serbia, which was then for somebody like me to survive, with freedom of speech, with an open mind, bright ideas, and brave by default. Early start to be in a positive centre of attention, open and free at my 15 to show as a woman and start dressing like that, even in school, getting good marks. Very early opportunities indicated that even to be a leader of many things, we as a group of youngsters with a willingness to do something for ourselves, used to really function. That was my practice to the end of college. Our generation represents the first outcomes of the individual rebellion against the monolithic system, the first self – oriented minds to come out of this area after more than 50 years. This outcome is not the perfect one, neither from the standpoint of the world’s interests, nor from their own. They are bitter and angry, more against than pro, frustrated and disappointed. Still, this generation is the most progressive force in the country, because it is an open – minded and tolerant generation for the most part. It is also a generation that is not afraid to act, probably the most important change compared to passive, obedient citizens of the socialist state. From our point of view, chance was there and waiting to be taken at the right moment. Then we start to open questions about sexualities. We didn’t have internet, we just heard that somebody’s cousin had a sex change. Get his (was she then, preparing for SRS) phone number and soon find the Belgrade, Gender Id Team and start continuously coming to psychiatric talks during high school. They prepare me well for real life after, and teach me to be a better human. In contemporary classifications, the term transsexualism has been replaced by the term gender disorder or gender dysphoria. The latter concept is more flexible and includes a wider range of conditions than the former concept of transsexualism. In the beginning, sex-change team - doctors have thought that it is 4 years possible to be just too intelligent and that trying to get SRS (1998) and women identity as a goal of living an easier life as a beautiful woman, could be one serious physical problem. The sex-reassignment surgery request was submitted on time, approved the same day my generation finished high school, and passed the admission tests for the Faculty of Forestry Engineering, and also because of the lack of a sexual biography at that time. Two years of taking female hormone therapy, then, fine skin, long, thick, dark hair with well feminine define body. At that time, at 18, it was enough to have mascara and high heels to pass the “extremely hot girl detector”, if it were some [1,14].

Anyhow, I convinced them of the sincerity of my intentions, and “it” began. (ends with breast augmentation in 1999 – Brasilia expertise, newer complied).

Happy as born, passing through all medical stages without any problems, was also my thing. The good thing about Serbia is that, at that time, we had the best experts (innovators) in that medical area, and social insurance was covering 90% of costs [15]. My father is a low expert, and he knew all the system “how to’s”, bureaucracy troubles to get all documents issued, and other necessary papers (diplomas, etc.) were much simpler. He helped me a lot, together with my whole family. The second difficult period was on the faculty, when everybody moved to the capital for studies. So happy that I had never lived in stealth. “That’s how she/he looks” was there again. This time, I was attracted to the boys I was with. That stringed warily hard, and it hurts nowadays. Being one of the best students again. We had a special night to end the faculty, something like a prom night. It is a custom to pronounce the prettiest girl from the forestry and agriculture faculty. That was me. (2002). I worked as a demonstrator, which is a kind of assistant professor on Forest Protection Cathedral, Faculty of Forestry.

Working in a public enterprise many years ago, as a forestry engineer, brought new stuff. That was in a small village near my hometown, and even if I had tried to stay in stealth, that would have been impossible. Forestry is, for the most part, man’s job, and they are so proud doing that macho stuffs it carries by itself. Green uniforms, every day field work, hunting, caring, ex, band saws, handling with big machines, mud, being whole day at ten below, and managing and controlling the field work of twenty half-drunk, strong-muscled forestry workers. I was blond then, in rubber boots and a green uniform, with perfect make-up and a French manicure, in a position to giving instruction and commands to such bunch. There were a lot of funny but struggling situations when they had not withdrawals to joke about everything surrounding them, and knowing everything about me. I stabilized that also, and I got estimableness and sympathy of many of them soon.

As mentioned above, rationally, 9 years after HRT start, it shows its full potential, so the patient „to be” is also like „live as a human being “! The only thing which is something suppose that is inevitably happen and is not possible to change is the libido through the forehead, this transformation. In particular, the almost perfect appearance of women in 9 years after the start of the use of oestradiol and progesterone, which later also implies the absence of testosterone, which the body no longer produces. With this, the magical hormone of vitality and sexuality in general, as well as general movement in general and unstoppable ability, the change of character goes to the point that the departure of libido and slowing down in every sense, does not always match the previous strength and perseverance of the personality. Then asexuality appears in a massive number of women, which is full of irony as a psychological response, even a depressive response to an almost impossible state for these psychological boosters. Summary/conclusion

This case report presents that sensation about belonging to the opposite sex biologically is one of the most powerful driving forces consider human will ever described and about what physiology meets at all, as discourse. Case confirms that if there is not a synchronous transformation of body with gender, or continuing life after puberty without gender identity and body transformation matching, this complex body and mind condition leads to “sexual death” of a human being and thus indirectly to the complete extinction of personality due to dysfunction at all levels. About 30 FTM & MTF ’s in Serbia, who finished their transition successfully, were participants in the questioning. Many of the „new women “live in „stealth “, and have no either high or elementary education, usually violently interrupted in many social integration trials to fit in before the whole process ends, resulting in „emigration “to a different social environment. From the other side, FTM’s are well professional, accomplished, and they are good doctors, engineers, or architects. They have children (by artificial fertilization) and usually get married, continuing with a good life, often in places where they were born. In Serbia, such men are accepted, even missing a Y chromosome, which the other group is unfortunately cursed with. This paper should be a good example that things could be different in the future, that modern society must accept the differences and even start helping vulnerable minorities with a collective consciousness. Humanity demands the next step that our civilization needs to take.
